Register a Kit

To register a GenoArt Creation Kit, please complete the registration form that is included in your kit and remember to include the complete form when you send  your samples to the laboratory for testing. 

 

When completing your form, you will select the theme, size and color choice for your GenoArt creation. Visit the CHOICES page for all of the available options.
